First Visit
The first visit typically lasts 60-75 minutes in order to get a detailed history, perform appropriate physical exams, order any labs needed, and give a detailed treatment plan. In my practice, I prefer to utilize multiple treatment modalities to effectively and creatively address the causes of your particular health concerns. Return visits typically last 30 minutes. Any acute visit for an established patient will typically last about 20-30 minutes.

Prior to your appointment there are forms that need to be filled out, the bulk of those are here, if you don’t fill these out prior to the appointment please come 15 minutes early to complete them.

Release of Records Form
Use this form if you have previous records at an office I have worked at in the past 5 years. For those of you who have released records to Total Health from a previous office, I have those. I will need you to release your records from the Total Health Center if you were seen there.
For new patients I often have you release records from previous health care providers, particularly labs, that you have seen in the past 5 years.

New Patient History
Please fill this out if you are new to the practice or have not seen me in 2 years.

New Pediatric Patient History
Please fill out for all patients under 16, unless you have filled one out in the past year.

Demographics and Insurance Information
All patients need one in the chart. I use the email for patient reminders so please include it in your demographics.
